ORDERING INFORMATION

Order Form Types:
• Program - for orders placed by the program with payment via a purchase order or credit card (includes student clinical tracking modules and CoARC survey system).
• Bookstore - for orders placed by the bookstore with payment via a purchase order or credit card (includes student clinical tracking modules and CoARC survey system).
• Student - for orders placed by individual students but only if the program rquires that students purchase a license directly from DataArc (includes student clinical tracking modules and CoARC survey system). Payment must be with credit card.
• Survey - only includes access to DataArc's online implementation of CoARC Survey instruments, records, and reports; does not include access to the various student clinical tracking modules; payment via purchase order or credit card.
